Add questions once per checkout

Attach one checkout form to cart orders and read its answers on the order.

Create the questions

Open Forms and choose the Checkout questions purpose. A question such as “How did you hear about us?” belongs here because it applies to the whole order. Keep preparation instructions for a particular item on its product form.

  1. Name the form and add supported question types.

  2. Mark an answer required only when customers must provide it before ordering.

  3. Save the form as active.

  4. Open Checkout settings, choose the form under Checkout questions, and save the settings.

What customers see

Customers answer the selected questions once per cart order. Required answers are checked before the order is accepted. This includes the first checkout for a customer subscription.

Digital-download checkout and Market Checkout's amount-only payment flow are separate. A checkout form does not become a public website form or replace product questions.

Read the saved answers

Open the order details to read its checkout answers. They are saved with the order, separately from each item's choices and instructions. They do not appear as website form submissions.

Change or remove the form

Use Checkout settings to select another active checkout form or remove the selection. Edit questions from Forms or the checkout settings shortcut. Existing orders retain their captured answers.

Common problems

  • The form is unavailable: choose an active form created for Checkout questions.

  • Questions are repeated on every item: move order-wide questions out of product forms.

  • An answer is missing from Forms submissions: open the order instead.
