Choose what the form is for
Open Forms and create a form from a template or start blank. Under What is this form for?, choose the purpose that matches where customers should answer.
- Extra product details: inscriptions, gift messages, preparation notes, or inspiration photos for an item.
- Checkout questions: information asked once for the whole cart order.
- Contact & custom order inquiries: requests you need to follow up on.
- Website form: other responses collected from a published page.
- Wholesale requests: business details and requested quantities for a quote.
Set flavors, sizes, quantities, and paid extras in Product Options. Use Reviews for new customer review collection.
Add useful questions
- Name the form so you can recognize it later.
- In Questions, add the fields customers need to complete. Keep labels short and mark only necessary answers required.
- Use Settings for form details, availability, response behavior, and notifications where supported.
- Save the form and read its Use this form instructions.
Connect the saved form
- Product: select it in the product's Product Forms section and save.
- Checkout: select it under Checkout questions in Checkout settings and save.
- Contact: select it under Form to display in the Contact page's Contact Form section.
- Website or wholesale page: add or edit a Form block, select the saved form, and save the page.
Creating a form does not publish it on a page. Page forms appear when connected to a published page. Blank pages require their own plan access.
Find responses
Product answers appear with order items. Checkout answers appear on the order details page. Page form responses use the form submission queue; wholesale requests can also lead to quote drafts in Orders. Keep these destinations separate when looking for an answer.
Before sharing
Open the relevant storefront page or product, check that the intended form appears once, and review required questions on mobile. If a form is unavailable, check its active status and saved connection.
