Attach a form to a product order

Add product-specific questions and choices to the ordering flow without duplicating products.

Attach during product creation

In Create product, some template flows show Forms used during ordering. Select one or more active product forms; they apply to every variation in that product.

Forms centralizes reusable customer intake and links directly to the submission queue.

Open the product editor from Products, or edit the form from Forms. Keep the form focused on information needed for that product rather than asking for duplicate contact details.

  1. Create or select an active Product form.
  2. Add fields such as radio, select, multiselect, date, image, or textarea.
  3. Use option pricing modifiers only when the price difference is intentional.
  4. Save the form and test every option in the storefront ordering flow.

What customers see

Product forms appear as part of the product order form. A required radio or select must be answered; multiselect fields can enforce minimums, maximums, and duplicate selections. Image fields can collect inspiration photos.

Common problems

  • The form does not appear: confirm it is Active and attached to the correct product.
  • Every variant shows the same form: product-wizard assignment applies to every variation; use conditions in Recipes & Costs only for formulation behavior, not to hide the form.
  • Option pricing is not reflected: enable pricing modifiers and verify the option values are stable.
  • A multiselect total is wrong: check whether duplicates are allowed and whether the configured min/max applies to total selections.
